Details of the Adult Social Care Grant 2009-2010 Paperwork

Attached is:

1. Copy of covering letter (pdf format)

2. Financial Form - you will need to arrange to complete and return this form (pdf/word format)

3. Claim Form - (word format) which also includes details of the claim process and eligibility.

I have also attached a copy of the Claim Form in excel format, as this may be easier for you to complete as it automatically calaculates the number of delegates and expenditure.

Within the Claim Form spreadsheet, we also have a short summary of course spreadsheet which we would ask that you complete and return, along with a delegate listing which can be completed and submitted with any in-house training claims.

You can arrange to submit the completed excel spreadsheet to grant@surreycare.org.uk with details of the claim form and short course summary.
