Following a review of the on demand availability of palliative care medication NHS Surrey decided to implement a new model of care incorporating services provided by Community Pharmacies and Thames Doc. The publication entitled On Demand Availability of Drugs for Palliative Care Service Description describes this service and provides detailed information on how It can be accessed.
